Description of resource: The Great Brook Valley Health Center is a comprehensive, multicultural, community health center with medical, dental, and mental health services, available to families and individuals of all ages. We pay special attention to chronic disease management, including HIV/AIDS and cardiovascular diseases. Our services include care at the health center, hospital, nursing home, and home visits. Care is coordinated with other local agencies such as home health and hospice.
